Job Overview
We are seeking a dynamic and compassionate Health Educator to join our team. In this role, you will develop and implement health education programs, provide personalized nutritional guidance, and collaborate with healthcare professionals to promote wellness among diverse populations. This paid position offers an exciting opportunity to make a meaningful impact on individuals’ health journeys through engaging education and support.

Responsibilities

• Conduct patient assessments to evaluate nutritional needs and develop tailored care plans
• Deliver nutrition counseling using evidence-based approaches, including motivational interviewing and medical nutrition therapy (MNT)
• Educate clients on chronic disease management, weight loss strategies, and dietary modifications for specific health conditions such as geriatrics or acute care needs
• Collaborate with healthcare professionals to coordinate comprehensive care plans and ensure optimal patient outcomes
• Maintain accurate documentation of client interactions, assessments, and progress within electronic health records (EHR/EHR systems)
• Supervise and mentor junior staff or interns in health education techniques and client communication strategies
• Stay current with the latest developments in clinical nutrition, physiology, and dietary support practices

Skills

• Extensive experience in health coaching, nutrition counseling, and clinical nutrition practices
• Strong knowledge of physiology, medical nutrition therapy (MNT), and dietary management for chronic diseases
• Proficiency with EMR/EHR systems for patient documentation and record keeping
• Skilled in supervising teams and providing effective client communication
• Ability to evaluate diagnostic data, perform nutritional analysis, and create comprehensive care plans
• Experience working with individuals with specific dietary needs across settings such as long-term care, hospital environments, or geriatrics
• Expertise in tube feeding management, weight loss programs, and acute care scenarios
• Familiarity with electronic health records (EHR) management and healthcare collaboration tools

Pay: $45.00 - $55.00 per hour
