Health Improvement Practitioner

The key purpose of the Health Improvement Practitioner (HIP) role in general practice is to work collaboratively as part of the wider primary care team, providing brief, consultation-based services to general practice patients, including evidenced-based behavioural interventions to patients of all ages, supporting both mental and physical health needs across individuals, groups and families.

To be actively engaged with utilising an integrated ‘Stepped Model of Care’ that supports patients’ needs and enables them to move seamlessly between services in general practice and, if needed, community-based NGOs and speciality services such as secondary mental health and addiction services.

To work with other Health Improvement Practitioners, behavioural health trainers, a service evaluator to refine the services delivered for the New Zealand context.

Candidates must hold current registration under one of the following:

· fully registered under the HPCA Act 2003, with a scope of practice that is appropriate to working as a mental health and addiction clinician or

· a registered alcohol and drug and/or gambling practitioner member of the DAPAANZ or

· a fully registered Social Worker or

· a full member of the NZ Association of Counsellors (NZAC).
