About the Role:

As a Health Improvement Practitioner at Te Kōhao Health, you will:

• Work within a general practice setting to deliver evidence-based behavioural interventions for individuals, groups, and whānau.
• Support clients' needs and transitions between different healthcare services.
• Collaborate with a multidisciplinary team to provide comprehensive care.

About You:

To excel in this role, you will possess:

• An annual practising certificate as a registered health professional.
• Clinical experience in mental health settings, providing evidence-based psychological interventions.
• Knowledge of behavioural medicine and the medical-psychological relationship.
• Demonstrated knowledge of Te Reo me ōna tikanga.
• Effective relationship management skills and an understanding of the healthcare system.
• The ability to connect with people from diverse backgrounds and cultures.
• Excellent communication, organisational, and planning skills.
