HHA/PCA

Duties

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary
• Monitor and observe patients’ conditions, including vital signs, medication adherence, and overall health status, documenting changes accurately in Electronic Health Record (EHR) systems
• Support clients with memory care needs, including Alzheimer’s and dementia care, ensuring a safe environment through vigilant patient observation
• Administer medications following medical documentation protocols while maintaining HIPAA compliance to protect patient privacy
• Assist with medical procedures such as catheter care, hospice support, and working with individuals with developmental disabilities or disabilities
• Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and safety standards
• Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment to promote overall well-being

Requirements

• Seeking female aide.
• Being able to speak English and Spanish mandatory.
• Prior experience in home health care, nursing homes, assisted living or long-term care settings preferred
• Knowledge of medical terminology, medication administration, vital signs monitoring, and basic medical documentation practices
• Additional certifications such as hospice care training or dementia/cognitive impairment training are a plus
• Ability to work independently while following detailed care plans and protocols
• Experience working with individuals with disabilities or developmental challenges is highly valued
• Strong communication skills to effectively collaborate with clients, families, and healthcare teams
