JOB DESCRIPTION

JOB TITLE: Attendant / Provider/Preferably Female

RESPONSIBLE TO: Supervisor

MAIN OBJECTIVE: To ensure quality and safe delivery of provider services that reflects

the agency’s philosophy and standards of care to assigned clients.

MINIMAL QUALIFICATIONS:

· At least 18 years of age.

· Experience in provider services or related health care field is preferred.

· Demonstrates ability to read and write and carry out direction.

· Written and verbal communication skills are necessary.

· Visual/hearing ability sufficient to comprehend written and verbal communications.

· Ability to perform tasks involving physical activity, which may include heavy lifting, extensive, bending and standing, turning and/or transferring patients weighing up to 150 pounds.

· Must have own transportation and be able to travel within the area covered by the agency.

· Attendance requires 8:00 to 5:00 generally, Monday to Friday, depending on the number of visits scheduled.

· Must have a driver’s license and auto insurance in accordance to state regulations.

· Demonstrates tact, patience and good personal hygiene.
