Duties

• Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and personal hygiene to promote dignity and independence
• Support patients with mobility using proper patient transfer methods and equipment like Hoyer lifts to ensure safety during movement
• Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals
• Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
• Administer medication following prescribed care plans while adhering to HIPAA regulations to maintain confidentiality and privacy
• Provide companionship and emotional support, fostering a positive environment that respects residents’ rights and promotes well-being
• Assist with light housekeeping tasks including cleaning, laundry, and maintaining a safe, sanitary environment in accordance with home & community care standards

Skills

• Proven experience working with individuals with disabilities, neurodevelopmental disorders, or in senior care environments such as assisted living or nursing homes
• Strong patient observation skills to detect changes in condition or behavior promptly and accurately
• Knowledge of dementia care, Alzheimer’s care, memory care, and hospice support practices
• Ability to perform manual handling tasks safely, including heavy lifting and patient transfers using appropriate techniques and equipment
• Familiarity with medication administration protocols and basic first aid/CPR certification for emergency response readiness
• Excellent communication skills for effective client interaction and coordination with healthcare teams
• Experience working within HIPAA guidelines to protect patient confidentiality and rights
