Duties

• Coordinate and schedule home care services, ensuring timely delivery of support from caregivers
• Serve as the primary point of contact for clients and their families, providing clear guidance and ongoing support.
• Monitor client progress regularly
• Collaborate with healthcare providers, social workers, and community resources to ensure comprehensive care delivery.
• Maintain accurate documentation of client interactions, service updates, and care plans within secure systems.
• Educate clients and families on available services, resources, and health management strategies to empower informed decision-making.
• Develop and maintain Caregiver schedules
• Match Caregivers with Participants
• Communicate shift details- Updates, schedule changes
• Document calls notes
• Train new caregivers on accessing and using scheduling system HHA exchange app or FOB
• Monitor Call dashboard linking any possible calls and making corrections if needed
• Prebilling- unbalance, authorization, temp, overlapping shifts, custom validations
• Answer all incoming calls
• Handle calls outs and restaff as needed
• Handle all authorization edits through Link communications

Skills

• Exceptional communication skills to effectively liaise with clients, families, and caregiving teams.
• Strong organizational abilities to manage multiple cases efficiently while maintaining attention to detail.
• Knowledge of home care services, healthcare regulations, and community resources is highly desirable.
• Spanish speaker is preferred
• Ability to remain flexible and proactive in responding to changing client needs.
• Empathy and compassion combined with a positive attitude to foster trusting relationships.
• Professional acumen and excellent customer service skills
• Problem-solving skills to address challenges swiftly and effectively while maintaining professionalism.
