**Home Care Intake Coordinator

$20–$25 per hour | Full-Time | In-Office Administrative Position** This is an in-office administrative job, not a caregiver or CNA position. No CNA license is required. You will not provide hands-on care. This role focuses on referral processing, insurance eligibility, enrollment coordination, and administrative follow-up.

Rockaway Home Care, a division of Foundation For the Elderly, is seeking a dependable and detail-oriented Home Care Intake Specialist to join our Dearborn, Michigan office.

You will serve as a primary point of contact for new referrals, helping consumers and their families navigate the home care enrollment process. You will coordinate with referral sources, insurance plans, Medicaid assistance resources, and internal teams to ensure applications are processed accurately and efficiently.

What You'll Do

• Receive and process new home care referrals by phone, email, and fax.
• Enter, review, and verify consumer information in our systems.
• Contact consumers and families to explain services and gather required documentation.
• Verify insurance coverage and eligibility for home care services.
• Coordinate referrals with Managed Long-Term Care (MLTC) plans and Medicaid assistance resources, as applicable.
• Follow up on insurance approvals and confirm authorized services.
• Ensure required authorizations and enrollment documentation are obtained for applicable cases.
• Communicate referral updates and case details to the Coordination Department.
• Follow up with consumers, families, insurance representatives, and referral sources to obtain missing information.
• Maintain accurate records, track pending referrals, and meet processing deadlines.
• Help ensure each consumer's enrollment process moves forward smoothly and efficiently.

What We're Looking For

• At least one year of home care intake experience required.
• Familiarity with home care intake procedures and MLTC requirements.
• Experience with Medicare, Medicaid, or insurance eligibility verification preferred.
• Strong verbal and written communication skills.
• Comfortable with data entry, Microsoft Office, and learning new computer systems.
• Organized, detail-oriented, and consistent with follow-up.
• Strong problem-solving and critical-thinking skills.
• Able to manage multiple tasks and meet deadlines.
• Dependable, professional, and comfortable working independently in an office environment.
