Home Health Aide

Duties

• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support using proper patient transfer methods and equipment such as Hoyer lifts.
• Monitor and document patient vital signs, health status, and changes in condition accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining strict patient confidentiality in accordance with HIPAA regulations.
• Support medication administration under supervision, ensuring proper dosage and timing while adhering to home healthcare regulations.
• Provide specialized care for clients with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques and compassionate engagement.
• Assist with medical procedures such as catheter care, wound dressing changes, and basic first aid or CPR in emergency situations.
• Prepare nutritious meals aligned with dietary restrictions and assist with light housekeeping tasks including cleaning and laundry to promote a safe living environment.
• Facilitate patient transfers using appropriate methods like gait belts or Hoyer lifts while ensuring safety protocols are followed at all times.
• Communicate effectively with clients’ families and healthcare teams to coordinate care plans and report any concerns or significant changes in health status.

Experience

• Prior experience working as a Home Health Aide or in assisted living facilities is highly preferred; experience working with individuals with disabilities or in geriatrics is a strong plus.
• Demonstrated knowledge of medical terminology, vital signs measurement, medication administration, and basic life support (CPR/First Aid).
• Familiarity with home health agency operations, hospice or end-of-life care settings is advantageous.
• Experience working within home & community care environments that require heavy lifting or the use of patient transfer equipment such as Hoyer lifts.
• Proven ability to maintain patient records accurately using EMR/EHR systems while respecting patient privacy laws like HIPAA.
• Strong communication skills for effective client interaction and collaboration with multidisciplinary teams.
• Valid certification in CPR/First Aid is required; additional certifications related to dementia or Alzheimer’s care are beneficial.

Pay: $23.13 - $27.85 per hour

Expected hours: 36.0 – 48.0 per week
