**THIS ROLE REQUIRES SKILLED CARE AND EXPERIENCE IS PREFERRED. ATTENTION TO DETAIL IS A MUST**

The available shifts Thursday - Saturday 7am-5pm and 2 overnight shifts (wake shifts) 8pm-8am.

Duties

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices
• Monitor patient health by taking vital signs and observing for any changes in condition or behavior
• Support medication administration following proper protocols and maintaining accurate medical documentation in Electronic Health Record (EHR) systems
• Assist with catheter care, wound care, and other complex medical needs as directed by healthcare professionals
• Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical guidelines - feeding tube is used
• The client has a tracheotomy which requires cleaning and maintenance.
• Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and organizing patient spaces
• Document all patient interactions accurately while ensuring compliance with HIPAA regulations to protect patient confidentiality

Experience

• Prior experience working in assisted living facilities, nursing homes, or home health environments preferred
• Demonstrated ability to work effectively with people with disabilities
• Familiarity with EMR (Electronic Medical Records) and EHR systems for accurate medical documentation
• Knowledge of medical terminology relevant to home health care settings
• Certification in CPR (Cardiopulmonary Resuscitation) required
• Experience handling medical equipment like Hoyer lifts or managing medication administration protocols preferred
• Long-term care experience or working within home & community care settings highly valued
