Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using patient transfer methods and equipment such as Hoyer lifts.
• Monitor patient health by observing vital signs, documenting medical information accurately in Electronic Health Record (EHR) systems, and reporting changes to healthcare professionals promptly.
• Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations and patient confidentiality standards.
• Provide specialized care for patients with dementia, Alzheimer’s disease, or other memory impairments, including memory care techniques and behavioral management strategies.
• Assist with medical procedures such as catheter care, wound dressing changes, and end-of-life care with compassion and professionalism.
• Maintain a clean and safe environment by performing light housekeeping tasks like cleaning, laundry, meal preparation, and ensuring infection control standards are met.
• Communicate effectively with clients, family members, and healthcare teams to coordinate care plans and ensure client satisfaction.

Qualifications

• Prior experience working in assisted living facilities, nursing homes, or home healthcare settings is highly preferred.
• Demonstrated skills in patient observation, medical terminology, vital signs measurement, and basic life support (BLS) or CPR certification.
• Knowledge of home health care assistance, hospice care practices, dementia care, geriatrics, and working with individuals with disabilities or developmental challenges.
• Familiarity with EMR/EHR systems for medical documentation and home health agency regulations.
• Ability to perform heavy lifting safely using proper techniques and equipment such as Hoyer lifts when necessary.
• Strong communication skills to effectively engage with patients and team members while maintaining patient confidentiality under HIPAA guidelines.
• Valid driver’s license and reliable transportation are often required for community-based visits.

Pay: From $14.50 per hour

Expected hours: 10.0 – 40.0 per week
