Duties

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature
• Administer medications accurately following prescribed schedules and medical guidelines
• Support patients with specialized care needs such as catheter management, dementia or Alzheimer’s care, and end-of-life assistance
• Maintain detailed medical documentation using Electronic Health Record (EHR) systems while ensuring compliance with HIPAA regulations to protect patient confidentiality
• Facilitate safe patient transfers utilizing methods like Hoyer lifts or patient transfer techniques to prevent injury
• Provide companionship and emotional support while engaging patients in activities that promote mental and physical well-being
• Follow home healthcare regulations and protocols to deliver high-quality care in a safe environment
• Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a clean living space conducive to health

Requirements

• Proven experience working as a Home Health Aide or in assisted living, nursing homes, or hospice settings
• Strong knowledge of medical terminology, patient care procedures, and home health care standards
• Ability to perform heavy lifting safely and operate equipment like Hoyer lifts or other mobility aids
• Certification in First Aid and Basic Life Support (BLS), including CPR training
• Experience working with individuals with developmental disabilities or dementia is highly preferred
• Excellent communication skills for effective client interaction and collaboration with healthcare teams
• Familiarity with EMR/EHR systems for accurate documentation of patient information
• Knowledge of home health agency operations and home & community care practices
• Ability to maintain patient confidentiality in accordance with HIPAA guidelines
• Valid driver’s license and reliable transportation are often required for travel between client homes
