Job Summary

We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients across diverse settings. Responsibilities

• Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support
• Monitor patient health by observing vital signs such as blood pressure, pulse, respiration, and temperature
• Administer medications accurately following prescribed instructions and maintain detailed medical documentation
• Support clients with mobility using patient transfer methods and equipment like Hoyer lifts or gait belts
• Provide specialized care for individuals with dementia, Alzheimer's disease, or other cognitive impairments, including memory care techniques
• Maintain patient confidentiality in compliance with HIPAA regulations while documenting care activities in Electronic Health Record (EHR) systems
• Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe environment
• Support end-of-life or hospice care patients with dignity and compassion during sensitive times
• Follow home healthcare regulations and safety protocols while delivering consistent patient care
• Communicate effectively with clients and their families to ensure understanding of care plans and address concerns

Requirements

• Proven experience working as a Home Health Aide or in assisted living/nursing home environments
• Demonstrated ability to perform patient observation and recognize changes in health status
• Knowledge of medical terminology, medication administration, catheter care, and vital signs monitoring
• Experience working with individuals with developmental disabilities or disabilities requiring specialized caregiving skills
• Certification in First Aid and Basic Life Support (CPR) is required; additional certifications such as hospice or dementia care are a plus
• Familiarity with EMR/EHR systems for medical documentation and client records management
• Ability to lift heavy objects safely using proper techniques; experience with patient transfer methods like Hoyer lifts is preferred
• Strong communication skills to effectively interact with patients, families, and healthcare teams
• Knowledge of home health care regulations and maintaining patient confidentiality under HIPAA standards
• Reliable transportation and flexibility to work various shifts including live-in employment options are desirable
