Home Health Aide

Job Post: Direct Support Professional (DSP) – Individualized Home Supports with Training (8 Hours/Week)
Seeking a compassionate, patient, and reliable Direct Support Professional to provide 8 hours per week of Individualized Home Supports with Training for an adult living in a Customized Living setting. This role focuses on promoting independence, meaningful engagement, community participation, and overall quality of life.

About the Individual

The individual enjoys humor, friendly people, and time outdoors. Fresh air is important for overall well-being. Interests include Bingo, crossword puzzles, singing, rock and roll music, documentaries, and classic TV shows. The individual also has a musical background and enjoys mentally engaging activities.

Position Goals

The support candidate will assist the individual in:

• Maintaining waiver eligibility and needed supports.
• Continuing to live safely in a Customized Living environment.
• Increasing community integration and participation.
• Engaging in meaningful activities such as walking, outings, music, puzzles, and reading.

Support Needs

Memory & Cognition

• Provide reminders, redirection, and encouragement to participate.

Community Integration

• Assist the individual in leaving their room and engaging in on-site or community activities.
• Support participation in Bingo, music events, outdoor walks, and outings with family.

Health & Safety

• Monitor for isolation, confusion, fatigue, or distress.
• Follow emergency protocols, including contacting designated supports and the case manager when needed.

Household Management

• Encourage participation in small tasks to promote independence.

Psychosocial Well-Being

• Provide companionship, conversation, and structured activities to reduce isolation.

Staff Expectations

• Prompt and encourage participation using patience and positive reinforcement.
• Offer simple choices to support independence.
• Engage the individual in activities that match personal interests.
• Promote decision-making skill development.
• Maintain awareness of safety and follow protocols as needed.

Ideal Candidate

• Calm, patient, and supportive.
• Ideally age 50s–60s to relate well with the individual.
• Enjoys activities such as walking, music, puzzles, and light social engagement.
• Comfortable providing prompts, encouragement, and gentle structure.
• Reliable and respectful of personal preferences and autonomy.

Pay: $21.00 - $22.00 per hour
