Company Description Elevate Infusion is a home infusion nursing service that provides skilled nursing coverage for specialty pharmacies across Missouri, Kansas, Arkansas, and Iowa. The organization focuses on closing nursing coverage gaps for complex therapies such as IVIG, SCIG, biologics, TPN, and central line care, including in rural and hard-to-staff areas. Elevate Infusion develops custom nursing solutions to help patients remain on therapy and receive timely care. Nurses practice under INS and IgNS standards and have experience managing high-acuity patients in the home setting. Services include home infusion nursing, home administration teach and train, hospital discharge teach and train, and central line/midline teach and train.

Role Description The Home Infusion RN role is a contract, on-site position based in Culbertson, NE, providing direct nursing care to patients receiving home infusion therapies. Day-to-day responsibilities include performing comprehensive patient assessments, initiating and monitoring IVIG, SCIG, biologics, and TPN infusions, and managing central line care in accordance with clinical protocols and safety standards. The RN will provide patient and caregiver education on medication administration, line care, symptom monitoring, and emergency procedures, including teach-and-train support for new starts and post-hospital discharge transitions. Documentation of assessments, interventions, patient responses, and communication with specialty pharmacies and prescribers is required to ensure continuity of care. The role also involves coordinating schedules, maintaining supplies, adhering to INS and IgNS standards of practice, and promoting a safe, high-quality home care environment.

Qualifications

• Strong clinical foundation in Nursing and Medicine, with the ability to assess, plan, implement, and evaluate patient care in the home setting.
• Experience in Home Infusion, including safe administration of IV and subcutaneous therapies, TPN, and management of central lines or midlines.
• Demonstrated competence in Direct Patient Care for high-acuity patients, with a focus on patient safety, comfort, and education.
• Background in Critical Care Medicine or acute care nursing, with familiarity in recognizing and responding to infusion-related complications.
• Active, unrestricted RN license in Nebraska (and ability to meet any multi-state licensure requirements as needed).
• Current BLS certification; ACLS preferred.
• Prior home health, home infusion, or specialty pharmacy collaboration experience is highly beneficial.
• Reliable transportation and ability to travel locally to patient homes; comfort working independently in community and rural environments.
• Excellent communication, documentation, and interpersonal skills, including the ability to educate patients and caregivers clearly.
