The decision to begin hospice care can be overwhelming for patients and families. At Concho Hearts Hospice, our Admissions Nurses help make that transition a little easier.

We're looking for a compassionate and confident Registered Nurse (RN) to join our Kilgore team as a Hospice Admissions Nurse.

This is a great opportunity for an RN who enjoys patient interaction, education, clinical assessment, and helping families understand what hospice care can provide.

What You'll Do

• Complete hospice admission visits in patients' homes, hospitals, facilities, and other care settings
• Perform comprehensive nursing assessments
• Educate patients and families about hospice services, medications, equipment, and what to expect
• Review and complete admission documentation and required consents
• Collaborate with physicians and the hospice interdisciplinary team
• Coordinate medications, DME, supplies, and other immediate patient needs
• Help ensure a smooth transition into hospice services
• Communicate important patient information to the RN Case Manager and clinical team
• Complete accurate and timely documentation in the EMR
• Provide reassurance and compassionate support to patients and families throughout the admission process

What We're Looking For

✔ Active Texas Registered Nurse (RN) license

✔ Strong nursing assessment and communication skills

✔ Hospice or home health experience preferred

✔ Comfortable educating patients and families about end-of-life care

✔ Organized and able to manage admission documentation efficiently

✔ Compassionate, dependable, and confident working independently

✔ Reliable transportation, valid driver's license, and current auto insurance

✔ Current CPR certification
