McLeod Health is seeking an experienced Nurse Practitioner to join McLeod Palliative Care Services. This community-based role provides compassionate care to patients with serious chronic and life-limiting illnesses through symptom management, advance care planning, and care coordination.

We are seeking a provider who resides in or is willing to relocate to the Horry County area, where a significant portion of our patient population receives care.

Position Highlights:

  • 100% outpatient, community-based practice with home-based patient visits

  • No call responsibilities
  • Average of 4-6 patient visits per day
  • Structured onboarding program with ongoing mentorship and clinical support
  • Collaborative interdisciplinary team environment
  • Opportunity to practice autonomously while supported by a highly experienced palliative care team

The provider will join an established team consisting of 3 physicians, 8 nurse practitioners, 5 RN care managers, 1 social worker, and dedicated administrative support. Responsibilities include conducting patient assessments, managing complex symptoms, facilitating goals-of-care discussions, evaluating hospice eligibility, and coordinating care across the healthcare continuum.
