About the Opportunity

American CareQuest, an accredited Home Health and Hospice agency, is seeking a compassionate and dedicated Hospice Chaplain to join our interdisciplinary care team.

We offer a flexible opportunity for experienced chaplains who are passionate about providing spiritual and emotional support to patients and families during end-of-life care.

This position is ideal for chaplains currently working in hospitals, hospice organizations, or other healthcare settings who are looking for additional hours and a flexible schedule.

Responsibilities

• Provide compassionate spiritual and emotional support to hospice patients and their families.
• Assess patients' spiritual needs and develop individualized spiritual care plans.
• Offer grief counseling, bereavement support, and end-of-life guidance.
• Support patients and families from diverse religious, spiritual, and cultural backgrounds.
• Collaborate with nurses, social workers, physicians, and other members of the interdisciplinary hospice team.
• Provide spiritual support during times of emotional distress, crisis, and end-of-life transitions.
• Maintain accurate and timely documentation of patient visits and services provided.
• Participate in interdisciplinary team meetings and care planning as needed.
• Coordinate with patients, families, and healthcare professionals to ensure compassionate, patient-centered care.

Qualifications

• Minimum of one year of experience in hospice, palliative care, hospital chaplaincy, or a related spiritual care setting preferred.
• Experience supporting patients and families facing serious illness, grief, or end-of-life concerns.
• Excellent communication, interpersonal, and organizational skills.
• Ability to provide compassionate and respectful care to individuals of diverse backgrounds and beliefs.
• Strong ability to work independently and collaborate with an interdisciplinary team.
• Reliable transportation and willingness to travel to patients' homes and healthcare facilities throughout the service area.
• Previous hospice experience is highly desirable.

Schedule and Flexibility

• Part-time and per diem opportunities.
• Flexible scheduling based on patient needs and availability.
• Field-based patient visits throughout San Francisco and San Mateo Counties.
• Opportunity to supplement an existing chaplaincy or healthcare position.
