Hospice Intake Coordinator

Faith Home Health and Hospice is seeking a detail-oriented and compassionate Hospice Intake Coordinator to join our team. This position plays a vital role in supporting patients, families, referral sources, and clinical staff by ensuring a smooth and timely admissions process for hospice services.

The ideal candidate will have prior hospice intake, admissions, or referral management experience and thrive in a fast-paced environment while maintaining exceptional customer service and attention to detail.

Essential Functions:

• Receive, process, and track patient referrals in the EMR system.
• Coordinate and communicate admission information with patients, families, referral sources, physicians, and clinical staff.
• Manage incoming phone calls, fax communications, and referral documentation.
• Prepare, assemble, and distribute admission packets and required regulatory paperwork.
• Track physician orders and ensure timely completion of documentation.
• Assist with quality assurance activities and regulatory compliance requirements.
• Verify referral information and maintain accurate patient records.
• Collaborate with interdisciplinary team members to facilitate seamless transitions of care.
• Provide excellent customer service while maintaining patient confidentiality and HIPAA compliance.

Qualifications:

• Minimum of one (1) year of hospice intake, admissions, referral coordination, or referral management experience required.
• Knowledge of hospice services, regulations, and admission processes preferred.
• Strong organizational skills with the ability to manage multiple priorities simultaneously.
• Excellent verbal and written communication skills.
• Proficiency in Microsoft Office applications, including Word and Excel.
• Ability to work independently and as part of a team.
• Valid driver's license and reliable transportation.
• Availability to participate in an alternating oncall weekend rotation.

Preferred Qualifications:

• Two (2) or more years of hospice experience
• Previous experience with electronic medical records (EMR) systems.
• Experience working in healthcare intake, admissions, or referral management role.
