As we continue to grow, we are seeking a passionate and driven Hospice Community Liaison to join our team. This position plays a vital role in expanding community awareness and strengthening relationships with referral sources to ensure patients and families have access to the care they need.

Position Overview

The Hospice Community Liaison is responsible for promoting Greater Help Hospice services through outreach, relationship-building, and education within the community and healthcare network. This individual will connect with physicians, hospitals, skilled nursing facilities, and other community partners to increase awareness of our services and support timely hospice referrals.

Key Responsibilities

• Develop and maintain strong relationships with referral sources, including physicians, hospital discharge planners, case managers, and nursing facilities.
• Promote Greater Help Hospice services to healthcare professionals and community organizations to increase referrals.
• Identify new business opportunities and create strategies to enhance community engagement and brand visibility.
• Conduct educational presentations and in-services for healthcare providers, patients, and families on hospice philosophy and benefits.
• Collaborate with the clinical and administrative teams to ensure a smooth transition for patients referred to hospice.
• Track marketing and outreach efforts, monitor referral patterns, and prepare performance reports for leadership.
• Represent Greater Help Hospice at community events, health fairs, and professional association meetings.

Qualifications

• High school diploma or higher
• Previous experience in healthcare sales, marketing, or business development — hospice or home health experience strongly preferred.
• Knowledge of hospice philosophy, Medicare, and Medicaid hospice regulations (preferred).
• Strong interpersonal, communication, and presentation skills.
• Self-motivated, organized, and able to manage multiple priorities independently.
• Working knowledge of Microsoft Office Suite (Word, Excel, PowerPoint).
• Valid driver’s license and reliable transportation required.
