Hospice Registered Nurse (RN)

Evergreen Health Care Management LLC is seeking a compassionate and experienced Registered Nurse (RN) to join our growing Hospice Care team in Northern Virginia.

We are looking for a dependable nurse who is passionate about providing high-quality, patient-centered end-of-life care. Full-time availability is strongly preferred, and candidates must also be willing to participate in an on-call rotation, including after-hours and weekend coverage as needed.

Key Responsibilities

• Provide skilled nursing care to hospice patients in their homes or other care settings.
• Complete initial and ongoing nursing assessments.
• Develop, implement, and update individualized plans of care in coordination with the interdisciplinary hospice team.
• Monitor patients' symptoms, comfort, pain management, and overall condition.
• Provide education and emotional support to patients and their families regarding end-of-life care.
• Coordinate care with physicians, hospice staff, caregivers, pharmacies, DME providers, and other healthcare professionals.
• Complete required clinical documentation accurately and within established timeframes.
• Perform routine, PRN, and other required nursing visits.
• Communicate changes in patient condition promptly to the appropriate members of the care team.
• Participate in interdisciplinary team meetings and care coordination.
• Assist with admissions and other clinical responsibilities as needed.
• Participate in the hospice on-call rotation and respond to urgent patient and family needs after regular business hours when scheduled.
• Maintain compliance with applicable hospice policies, procedures, and regulatory requirements.

Qualifications

• Current and unrestricted Registered Nurse (RN) license in Virginia.
• Graduate of an accredited nursing program.
• Previous experience in Hospice, Home Health, Palliative Care, or a related clinical setting is preferred.
• Hospice experience is highly preferred.
• Strong clinical assessment, communication, and documentation skills.
• Ability to work independently in a home-based care environment.
• Compassionate and professional approach when working with patients and families experiencing end-of-life situations.
• Reliable transportation and ability to travel to patient locations.
• Valid driver's license and appropriate automobile insurance, as applicable.
• Ability and willingness to participate in an on-call schedule, including evenings, nights, weekends, and/or holidays as assigned.

Schedule

• Full-Time Preferred
• Monday through Friday / schedule based on patient care needs
• On-call availability required
• Some evenings, weekends, and holidays may be required as part of the on-call rotation
