Description:

a Spirit Hospice Nurse Case Manager balances expert-level compassion with a sense of vibrancy for life (yes, we laugh andcry here). If you're a pro at navigating end-of-life care with grace and empathy, you might be our kind of people. You'll be the clinical anchor for patients and families, helping them feel supported, informed, and genuinely cared for every step of the way. We value teamwork, communication, snacks in the breakroom during IDT, and most importantly heart. If you’re passionate about providing exceptional care while working with a team that actually utilizes teamwork, we’d love to meet you.

Position Requirements: 

• Current RN license in the State of Georgia
• Current CPR & First Aid Certification
• Reliable and legal transportation to and from patient's place of residence
• Hospice or home care experience is a plus, but kindness and reliability matter the most
