Three Rivers Hospice is seeking a compassionate, dedicated, and skilled Registered Nurse (RN) Case Manager to join our team in Warrensburg, Missouri, serving patients and families throughout Warrensburg and the surrounding communities.

As a Hospice RN Case Manager, you will have the opportunity to provide meaningful, one-on-one care while building lasting relationships with patients and their families. You'll play an essential role in ensuring patients receive the comfort, dignity, and compassionate care they deserve during life's most important moments.

What You'll Do

• Provide comprehensive nursing care to hospice patients in their homes and nursing facilities.
• Develop, implement, and coordinate individualized plans of care in collaboration with the interdisciplinary team.
• Perform ongoing patient assessments and adjust care to meet changing needs.
• Manage pain and symptoms to promote patient comfort and quality of life.
• Educate and support patients and families throughout the end-of-life journey.
• Collaborate with physicians, caregivers, and other healthcare professionals to ensure coordinated care.
• Maintain accurate, timely documentation of patient assessments, interventions, and outcomes.
• Advocate for patients and families while respecting their wishes, values, and goals.

What We're Looking For

• Active Missouri RN license
• Minimum of 1–2 years of nursing experience; hospice or home health experience preferred.
• Strong clinical assessment, critical thinking, and organizational skills.
• Excellent communication and documentation abilities.
• Ability to work independently while remaining an active part of a supportive team.
• A compassionate, dependable, and patient-centered approach to nursing.
• Valid driver's license, reliable transportation, and current auto insurance.
