Hospice RN Case Manager PRN

Overview
We are seeking a dedicated and compassionate Registered Nurse Case Manager PRN to join our healthcare team. In this pivotal role, you will be responsible for coordinating patient care, ensuring effective communication among healthcare providers, and advocating for patients' needs. Your expertise in case management will help streamline processes and enhance patient outcomes across various settings, including emergency medicine, clinics, geriatrics, pediatrics, and intensive care units (ICU).

Responsibilities

DESIRED RN WILL BE WORKING PRN IN ATLANTA AREA TAKING CARE OF OUR HOSPICE PATIENTS AS NEEDED.

• Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans.
• Coordinate discharge planning to ensure smooth transitions from hospital to home or other care facilities.
• Collaborate with multidisciplinary teams to facilitate effective communication and continuity of care.
• Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
• Provide education and resources to patients and families regarding health management and available services.
• Engage in utilization management to optimize resource allocation while maintaining high-quality patient care.
• Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.
