What they need

Job Overview

Are you passionate about making a meaningful difference in people's lives? As a Host Home Provider, you will serve as a compassionate and dedicated caregiver, offering a safe, supportive, and nurturing home environment for individuals with diverse needs. This rewarding role involves providing personalized care, assisting with daily activities, and promoting independence for residents. You will play a vital part in enhancing quality of life while fostering a warm, welcoming atmosphere that encourages growth and well-being. This is an excellent opportunity for motivated individuals eager to contribute to their community through caregiving and support.

Responsibilities

  • Provide comprehensive personal care assistance, including help with bathing, dressing, grooming, and mobility.
  • Assist residents with activities of daily living (ADLs) such as meal preparation, medication administration, and personal hygiene.
  • Monitor residents’ health by observing vital signs, tracking care plans, and reporting any changes to healthcare professionals.
  • Support residents in engaging with community activities and facilitate transportation as needed.
  • Maintain a clean, safe, and organized living environment by performing regular cleaning duties and adhering to safety standards.
  • Follow HIPAA guidelines to ensure confidentiality and privacy of all resident information.
  • Use manual handling techniques and patient transfer methods such as Hoyer lifts safely to assist with mobility needs.
  • Administer medications accurately according to prescribed care plans while maintaining detailed documentation.
  • Observe residents closely for signs of dementia, Alzheimer's disease, or other neurodevelopmental disorders; implement specialized memory or dementia care strategies.
  • Ensure residents’ rights are upheld at all times by respecting their preferences and promoting independence.
  • Collaborate with healthcare teams to develop and follow individualized care plans tailored to each resident’s needs.
  • Document all patient interactions thoroughly in compliance with healthcare regulations and home health standards.

Experience

Candidates should possess prior experience working in group homes, assisted living facilities, nursing homes, or similar settings supporting individuals with disabilities or aging populations. Experience working with individuals with autism, developmental disabilities, or neurodevelopmental disorders is highly valued. Familiarity with patient monitoring techniques such as vital signs measurement and medication administration is essential. Candidates should demonstrate proficiency in caregiving tasks including assisting with ADLs, using Hoyer lifts or patient transfer methods, and providing dementia or Alzheimer’s care. Prior roles involving home health services or hospice care are advantageous. A background in geriatrics or long-term care environments will support success in this role. Certification in CPR and first aid is required; additional training in HIPAA compliance and manual handling techniques is preferred. Strong communication skills for client interaction and team collaboration are essential for delivering exceptional resident-centered care.
