ID Case Manager

Duties

• Develop individualized service plans tailored to each client's unique needs and goals, emphasizing independence and community integration
• Coordinate access to essential resources such as housing, healthcare, employment, and behavioral health services
• Conduct regular assessments to monitor progress, adjust plans, and address emerging challenges promptly
• Utilize motivational interviewing techniques to motivate clients toward positive behavioral changes and goal achievement
• Manage crisis situations effectively through crisis management strategies, ensuring safety and stability for clients
• Collaborate with community partners to facilitate Section 8 housing applications and other workforce development initiatives
• Maintain detailed documentation of client interactions, service plans, and progress notes in compliance with agency policies

Requirements

• Strong skills in motivational interviewing and crisis management techniques
• Ability to work independently while maintaining a collaborative team approach
• Excellent communication skills to engage clients effectively and advocate on their behalf
• Strong organizational and multi-tasking skills
• A valid driver’s license and reliable transportation for client visits and community outreach activities

Pay: $23.00 - $25.00 per hour
