Independent RN Contractor

Job Summary

The Independent Registered Nurse (RN) is responsible for conducting comprehensive nursing assessments and developing, implementing, and monitoring individualized care plans for elderly clients. The RN will ensure that client documentation remains current and supports the client’s health, safety, well-being, and individual needs.

Key Responsibilities

• Conduct comprehensive RN assessments of clients upon admission, reassessment, or as otherwise required.
• Develop individualized care plans based on each client’s medical, functional, psychosocial, and safety needs.
• Prepare and maintain Individual Abuse Prevention Plans (IAPP) for elderly clients, identifying individual risks and appropriate prevention and protection strategies.
• Complete and update monthly progress notes, documenting changes in condition, services, needs, and outcomes.
• Conduct quarterly reviews of client care plans and IAPPs and make revisions as needed based on changes in the client’s condition or circumstances.
• Conduct an annual review of the client’s social history and update documentation to reflect significant changes.
• Communicate relevant changes in client condition or needs to appropriate caregivers, providers, family members, or the supervising organization, as permitted and required.
• Maintain accurate, timely, confidential, and compliant clinical documentation.
• Identify changes in health status, safety risks, functional abilities, or support needs and recommend appropriate interventions or follow-up.
• Ensure assessments, care plans, IAPPs, progress notes, and other documentation are completed within required timelines.
• Follow applicable federal, state, local, professional, privacy, and elder-abuse prevention requirements.

Qualifications

• Valid RN license with current registration in the applicable state.
• Proven experience working in diverse settings such as hospitals (Level I & II trauma centers), ICU, PICU, NICU, emergency rooms, outpatient clinics, long-term care facilities, assisted living communities, hospice or palliative care environments.
• Strong proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or similar platforms.
• Extensive clinical skills including IV insertion and infusion therapy, suturing, phlebotomy, specimen collection/processing, and infection control practices.
• Critical care experience with familiarity in trauma medicine, cardiac catheterization procedures, ventilator management and acute pain management.
• Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), CPT coding for procedures and treatments.
• Experience working with individuals with disabilities or developmental delays is highly desirable.
• Ability to perform comprehensive patient assessments across various age groups including pediatrics (neonatal resuscitation procedures) and geriatrics (memory care).
• Excellent communication skills for effective collaboration within multidisciplinary teams and providing health coaching when needed.
