Individualized Home Support (IHS) with Training/Homemaker – Part-Time (25 Hours/Week

Position Summary

We are seeking a compassionate and reliable Individualized Home Support (IHS) with Training Staff to provide one-on-one support to adults receiving HCBS services in their home and community. The goal of this role is to help individuals learn and strengthen daily living skills that promote independence and personal growth.

Responsibilities

• Provide training and support in areas such as cooking, budgeting, communication, and personal organization.
• Support individuals with community integration, including transportation to appointments, shopping, and recreational activities.
• Follow each person’s individual support plan (ISP) and document progress per DHS 245D requirements.
• Promote independence, dignity, and safety in all activities.
• Communicate effectively with clients, supervisors, and case managers to ensure high-quality care and compliance.

Qualifications

• Must be 18 years of age or older.
• High school diploma or GED required.
• Experience in disability services, HCBS, PCA, ILS, DSP, or related field preferred.
• Must pass a Minnesota DHS background study.
• Valid driver’s license and reliable transportation required.
• Compassionate, dependable, and able to work independently.
