Inpatient Medical Coders | IP Coders | Inpatient Coding

We are looking for skilled Inpatient Medical Coders to join our Healthcare BPO team. You will be responsible for accurately reviewing inpatient medical records and assigning appropriate diagnosis and procedure codes based on official coding guidelines, documentation, and payer requirements.

Key Responsibilities

• Review inpatient medical records, clinical documentation, discharge summaries, operative reports, and other relevant documentation.
• Assign accurate ICD-10-CM and ICD-10-PCS codes based on provider documentation.
• Apply official inpatient coding guidelines and established coding policies.
• Ensure diagnoses and procedures are appropriately supported by clinical documentation.
• Identify and resolve documentation or coding discrepancies.
• Abstract relevant patient information from medical records into designated systems.
• Review charts for completeness and coding accuracy.
• Maintain required quality, productivity, and turnaround-time standards.
• Participate in quality audits, coding reviews, and feedback sessions.
• Research coding guidelines and regulatory updates when necessary.
• Escalate unclear or insufficient documentation through the appropriate channels.
• Maintain strict confidentiality when handling patient health information and protected health information (PHI).
• Stay updated on changes to ICD-10-CM, ICD-10-PCS, official coding guidelines, and healthcare regulations.
• Collaborate with clinical documentation, quality, audit, and operations teams as needed.

Qualification

• 1+ year of inpatient medical coding experience preferred.
• Strong knowledge of ICD-10-CM and ICD-10-PCS coding systems.
• Working knowledge of inpatient coding guidelines and conventions.
• Strong understanding of medical terminology, anatomy, physiology, and disease processes.
• Ability to accurately interpret clinical documentation.
• Strong analytical and attention-to-detail skills.
• Excellent written and verbal English communication skills.
• Proficient in computer applications and electronic medical record systems.
• Ability to meet productivity, quality, and turnaround-time targets.
• Strong organizational and time-management skills.
• Willing to work on a night shift or shifting schedule, depending on business requirements.
