Now Hiring: Spanish-Speaking Intake Coordinator

Location: Hempstead, NY
Job Type: Full-time, starting part-time
Pay: $25–$29 per hour (approximately $27 per hour)

Suffolk Primary Health is seeking a warm, dependable, and detail-oriented Spanish-speaking Intake Coordinator to join our care management team serving Long Island. This role is a good fit for someone who enjoys helping families navigate services, coordinating care, and solving problems.

During the initial months, you’ll also serve as a Care Manager, providing support to members and families, including monthly home visits.

What You’ll Do

• Manage intake by reviewing referrals, determining eligibility, and coordinating enrollment
• Conduct monthly home visits and provide ongoing support to members and families
• Coordinate with families, providers, therapists, community organizations, and Medicaid Managed Care Plans
• Conduct assessments and develop individualized care plans
• Maintain complete, accurate, and timely documentation
• Follow applicable New York State Department of Health, Health Home, and regulatory requirements
• Attend weekly virtual trainings, team meetings, and program updates
• Maintain information about services, providers, and community resources to help guide referrals

What We’re Looking For

• Bachelor’s degree required
• Spanish bilingual skills required
• Strong organizational, communication, and computer skills
• Dependable, detail-oriented, and comfortable working independently and with a team
• Comfortable communicating with families and navigating paperwork
• Strong problem-solving skills and a proactive approach
• Experience in healthcare, social services, psychology, case management, government programs, or insurance is a plus
