Intake Specialist

Core Virtual Solutions is hiring an Intake Specialist.

KEY RESPONSIBILITIES

• Call patients monthly and complete a 20-minute CCM intake
• Document symptoms, medications, adherence, care-plan goals, labs/imaging, referrals, and ER/hospitalizations
• Document encounters and actual CCM time in NextGen
• Coordinate follow-ups, referrals, and medication/refill concerns within scope
• Escalate clinical concerns to the provider/RN
• Prepare completed monthly CCM documentation for billing

QUALIFICATIONS

• Medical Assistant, Clinical VA, or similar healthcare experience
• Hands-on experience with patient calls and medical documentation
• EHR/NextGen experience preferred
• Strong communication and documentation skills
• Rheumatology or Chronic Care Management experience is a plus
