Integrated Care Extender

We are seeking an Integrated Care Extender to support care coordination efforts for clients with developmental, behavioral health, or related needs.

In this role, you will assist care managers by conducting outreach, engaging clients, coordinating services, and tracking referrals . Your responsibilities include ensuring clients stay connected to resources, providing health education, assisting with appointments and transportation, and maintaining accurate documentation within the care management platform.

Role Ideal candidates will have a minimum of two years' experience in similar roles , with at least one year working with the Tailored Care Management population, and meet one of the following criteria:

• Personal experience navigating Medicaid or with I/DD/TBI;
• Certified Peer Support Specialist with behavioral health experience;
• A parent or guardian with direct care and navigation experience (excluding care for their own family members).

They should also be at least 18 years old with a high school diploma or equivalent.

Qualifications include strong relationship-building skills, knowledge of community resources, and the ability to collaborate effectively with multidisciplinary teams.
