ITC COORDINATOR (INTEGRATED TEAM CARE)

JOIN YADU AND EXPERIENCE A FULFILLING CAREER AND AN UNMATCHED LIFESTYLE!

About Yadu Health

Yadu Health Aboriginal Corporation is a not-for-profit, community-controlled health service providing primary health care to Aboriginal communities in Ceduna, Koonibba, and surrounding areas. Operating across four sites, we deliver culturally safe and supportive services through a multidisciplinary team of GPs, nurses, visiting specialists, and Aboriginal health workers focused on improving community health and wellbeing.

Life in Ceduna: Ceduna isn’t just a place to work, it’s a place to live, explore, and thrive.

  • Coastal Beauty: Pristine beaches, breathtaking sunsets, and endless fishing, surfing, and boating opportunities.
  • Outdoor Adventure: Experience the best of nature, from whale watching at the Great Australian Bight to exploring the stunning Nullarbor Plain.
  • Affordable & Relaxed Living: Enjoy a cost-effective lifestyle with housing and living costs far lower than in major cities.
  • Welcoming Community: A friendly and diverse town where you’ll feel right at home.

About the Role

The ITC Coordinator is a pivotal role in Yadu Health’s Integrated Team Care (ITC) program and is responsible for improving health outcomes for Aboriginal people with chronic health conditions through better access to coordinated, multidisciplinary team care, including clinical support. Based in Ceduna, you will drive the Care Coordination Model at the local level, using existing infrastructure and partnerships to link clients with the services they need and supporting them to better self-manage their chronic disease. You will bring a strong commitment to culturally respectful and safe practice in all dealings with Aboriginal clients.

Key responsibilities include (but not limited to):

  • Drive the implementation of the Care Coordination Model at the local level, supporting Aboriginal people with chronic disease and complex care needs to access clinical and community support services tailored to their needs.
  • Work closely with GPs and clinic staff to coordinate health care plans, action referrals and recalls, and support clients to implement their GP Management Plans and Team Care Arrangements.
  • Improve timely access to allied health, specialist and transport services, including coordinating travel bookings for clients to attend appointments locally, across the Eyre Peninsula, Port Augusta and Adelaide.
  • Act as a cultural broker between clients, their families and health service staff, and work collaboratively with Care Coordinators, Aboriginal Health Project Officers, Aboriginal Outreach Workers, community groups and other agencies.
  • Maintain accurate and timely client records and data in Communicare, and contribute to quality, accreditation and funding body reporting requirements.

This role requires someone who is proactive, culturally respectful, and deeply aligned with our mission to support First Nations health outcomes.

About You

To thrive in this position, we are looking for candidates who can meet the following criteria:

Essential:

  • A relevant health qualification as a Registered Nurse, Enrolled Nurse, Aboriginal and Torres Strait Islander Health Practitioner or Aboriginal Health Worker.
  • Current AHPRA registration.
  • Sound knowledge of chronic disease management and care planning in a primary health care setting.
  • Experience in chronic disease care coordination or primary health care, ideally within an Aboriginal Community Controlled Health Service.
  • Solid understanding of the health system and referral pathways, and the ability to work collaboratively with GPs, specialists, nurses and allied health professionals.
  • Experience working with people from Aboriginal communities, and the ability to provide culturally safe care and to advocate for Aboriginal and Torres Strait Islander clients and their families.
  • Strong organisational and computer skills, including accurate record keeping in electronic clinical systems.
  • Demonstrated ability to work independently, assess priorities and meet deadlines.
  • Valid Driver Licence (Class C or equivalent).
  • Have permanent work rights in Australia.

Desirable:

  • Strong relationships with the local Aboriginal and Torres Strait Islander Community.
  • Experience in the implementation, evaluation and delivery of health and education programs.
