What they need
Role Description: As a Lead Nurse, you will have the unique opportunity to combine hands-on clinical care with leadership and mentorship in a meaningful, relationship-centered environment. This salaried role is designed for an experienced RN who is passionate about delivering exceptional one-on-one care while helping shape clinical excellence across our organization.
The primary focus of the role is providing hands-on nursing care and clinical oversight for an individual client, developing personalized care strategies that maximize outcomes, independence, and quality of life in the home setting. In addition to this primary clinical focus, you will support the broader nursing team through RN assessments, supervisory visits, care plan development, and clinical evaluations.
Lead Nurses participate in a shared on-call rotation, offering remote clinical guidance and support to caregivers, clients, and nurses. This is an ideal opportunity for a nurse seeking the rewarding balance of meaningful client relationships, clinical autonomy, and leadership impact.
Key Responsibilities:
Skilled Focus:
- Perform thorough patient/client initial assessments and reassessments, evaluating the client's medical needs and developing the appropriate Plan of Care, supporting documentation/tools, and care strategies.
- Provide respite care for medically fragile patients, ensuring their comfort and well-being.
- Conduct 62-day supervisory visits to assess patient progress and ensure adherence to care plans.
- Participate in alternating after-hours on-call phone duty with other nursing staff to address urgent patient needs.
- Manage electronic medical records (EMRs) to maintain accurate and up-to-date patient information.
- Fulfill LVN/RN duties as needed, serving as a role model and mentor for the nursing team.
Case Management:
- Oversee nursing case management to optimize patient outcomes and promote efficient care delivery.
- Coordinate and lead the care staff to ensure smooth operations and efficient patient care.
- Act as a liaison between doctors and clients, facilitating effective communication and collaboration.
- Deliver training sessions for staff and clients, promoting best practices and ensuring compliance with healthcare standards.
- Handle any other duties assigned by management to support the smooth functioning of our healthcare facility.
Qualifications
- Licensed Registered Nurse (RN) in the state of California, with at least one year of experience in home health, skilled nursing, or community-based care.
- Strong clinical assessment and care planning skills, including the ability to manage complex cases and coordinate interdisciplinary care.
- Leadership and team coordination abilities, with experience supervising or mentoring nurses and caregivers, are a plus!
- Excellent communication and interpersonal skills for collaborating with clients, families, and healthcare partners in diverse communities.
- Knowledge of infection control, patient safety practices, and applicable regulatory and documentation standards.
- Ability to work on-site in clients’ homes and community settings with reliable transportation and a clean driving record.
- Comfort with electronic health records or clinical documentation systems, and basic proficiency with common office software.
- CPR certification and other relevant clinical credentials; prior experience in hospice, chronic illness management, or post-acute care is a plus.
- Ability to successfully pass a comprehensive background check, physical, and TB screening to ensure patient safety and welfare.
- Ability to lift up to 50 pounds.
- Comfortable with bending, standing, and navigating stairs.
