Licensed Clinical Social Worker

Overview

• Provide clinical social work services in a skilled nursing facility, supporting rehabilitation for short-term and long-term residents.
• Help residents and their families navigate care needs, adjustment, and transitions while collaborating with the interdisciplinary team.

Responsibilities

• Assess residents’ psychosocial needs and develop individualized care plans in coordination with residents, families, and care team members.
• Provide counseling, emotional support, and referrals to appropriate community resources.
• Support discharge planning and care transitions for short-term and long-term residents.
• Maintain accurate, timely documentation and communicate relevant updates to the interdisciplinary team.

Skills

• Active Licensed Clinical Social Worker (LCSW) credential required.
• Strong assessment, counseling, care-planning, and communication skills.
• Ability to collaborate with residents, families, and interdisciplinary care teams.
• Sound judgment, empathy, and organization in a skilled nursing facility setting.

Pay: $20.00 - $23.00 per hour

Expected hours: 40.0 per week
