What they need

Olive Branch Muslim Family Services is seeking a compassionate, organized, and community-focused LPC Associate Counselor / Case Manager & Outreach Coordinator to provide counseling, case management, resource navigation, referral coordination, and community outreach services.

This position works directly with individuals and families to assess needs, provide counseling within the scope of licensure and clinical supervision, connect clients with appropriate resources, and ensure referrals and follow-up services are completed thoroughly and in a timely manner. The position also supports community outreach by identifying individuals and families who may benefit from services, strengthening partner relationships, and increasing awareness of Olive Branch programs.

Olive Branch serves Houston's Muslim community as well as the broader community and focuses on strengthening families, promoting self-sufficiency, and improving clients' social and economic well-being.

Counseling & Clinical Responsibilities

  • Conduct client intake interviews and assessments to identify counseling, psychosocial, and supportive-service needs.
  • Provide individual counseling and psychotherapy within the scope of professional licensure and clinical supervision.
  • Develop and maintain individualized treatment plans; monitor client progress and adjust goals as appropriate.
  • Maintain accurate and timely progress of notes, assessments, treatment plans, and other required clinical documentation.
  • Coordinate appropriate referrals to specialized mental health or community-based services when needed.
  • Support discharge, transfer, continuity-of-care planning, and appropriate crisis response in accordance with organizational procedures and professional standards.
  • Maintain confidentiality, professional boundaries, and all applicable ethical and legal standards.

Case Management & Referral Coordination

  • Provide ongoing case management and resource-navigation support to clients and families.
  • Assess client needs and connect clients with appropriate internal Olive Branch programs and external community resources, including employment, healthcare, childcare, food assistance, housing, public benefits, education, workforce development, and mental-health services.
  • Ensure all referrals are completed accurately and followed through until the client is connected to the appropriate service, or the referral is formally closed.
  • Follow up consistently with clients regarding referrals, appointments, applications, and outstanding needs.
  • Communicate with partner organizations and service providers when appropriate and authorized, and identify alternatives when barriers prevent access to services.
  • Maintain accurate documentation of referrals, follow-ups, outcomes, unresolved needs, case records, client tracking systems, and organizational spreadsheets.
  • Ensure client cases are not left without appropriate follow-up or documentation.

Community Outreach & Client Engagement

  • Conduct community outreach to increase awareness of Olive Branch programs and services and identify individuals and families who may benefit from them.
  • Follow up with prospective clients and community referrals.
  • Build and maintain relationships with nonprofit organizations, schools, healthcare providers, faith-based organizations, social-service agencies, and other community partners.
  • Develop and strengthen referral relationships that can connect clients to Olive Branch services.
  • Attend community events, resource fairs, meetings, workshops, and outreach activities as a representative of Olive Branch.
  • Provide community partners with accurate information regarding Olive Branch services and referral processes.
  • Support outreach activities designed to increase client participation, strengthen partnerships, and identify new community resources.

Administrative & Organizational Responsibilities

  • Maintain timely and accurate documentation in Olive Branch's approved systems.
  • Participate in staff meetings, clinical supervision, case consultations, training, and organizational meetings as required.
  • Communicate regularly with the direct supervisor regarding caseload, referrals, outstanding client needs, documentation, and operational concerns.
  • Assist with client tracking, program reporting, and outcome documentation as requested.
  • Follow Olive Branch policies, procedures, workflows, confidentiality requirements, and documentation standards.
  • Complete assigned tasks and follow-ups within established deadlines and provide reasonable office and program support when needed to ensure continuity of client services.
  • Perform other duties and responsibilities as assigned by the direct supervisor.

Qualifications

  • Master's degree in counseling, Clinical Mental Health Counseling, Psychology, or related behavioral health field.
  • Current Texas credential/license in good standing as an LPC Associate, LMFT Associate, LMSW, LPC, LMFT, or LCSW.
  • Strong counseling, case management, resource-navigation, client engagement, documentation, organizational, and follow-up skills.
  • Ability to work effectively with individuals and families from diverse cultural and socioeconomic backgrounds; cultural competency and familiarity with Islamic faith, traditions, and communities strongly preferred.
  • Strong written and verbal communication skills and the ability to manage multiple clients, referrals, deadlines, and priorities.
  • Strong attention to detail and commitment to following cases through completion.
  • Comfortable conducting community outreach and developing professional relationships.
  • Proficiency with electronic documentation systems, online resources, and standard office applications.
  • Previous experience in nonprofit, community-based, behavioral-health, or social-service settings preferred.
