• Conduct comprehensive assessments of residents' needs, identifying areas for improvement and proposing recommendations for optimal care delivery
• Coordinate and manage MDS (Minimum Data Set) assessments, ensuring accuracy and timeliness of submissions
• Maintain accurate and up-to-date resident records, ensuring compliance with regulatory standards and internal policies
• Participate in quality improvement initiatives, contributing to the development and implementation of strategies to improve resident outcomes and satisfaction
• Provide education and support to staff members, families, and residents, promoting a culture of open communication and compassion
