Licensed Master Social Worker

Job Title: Licensed Master Social Worker (LMSW)

Location: New York, NY 10016

Contract Duration: 13 weeks + probable extension

Shift: Day Shift (9:00 AM – 5:00 PM, Monday–Friday)

Pay Rate: $61/hr - $64/hr

Required Qualifications

• Master's Degree in Social Work (MSW) – Required
• Active New York State LMSW License – Required
• Minimum 1 year of LMSW experience
• Experience in inpatient medical/surgical settings preferred.
• Strong knowledge of discharge planning, patient assessments, and care coordination.
• Excellent communication and interpersonal skills.
• Proficiency in electronic medical records (EMR).

Responsibilities

• Attend interdisciplinary team meetings and unit rounds.
• Conduct patient screenings, psychosocial assessments, and care planning.
• Develop comprehensive and safe discharge and aftercare plans in collaboration with the interdisciplinary team.
• Coordinate discharge planning, including referrals to home care agencies and skilled nursing facilities (SNFs).
• Meet with assigned patients at least once per week, unless clinically inadvisable.
• Complete assessments, progress notes, care plans, and patient education documentation within required time frames.
• Utilize Allscripts, CarePort, and Parachute applications for referrals and orders.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure continuity of care.
