Position Summary

Altra Home Health is looking for caring, skilled nurses to join our team as RNs and LPNs. You’ll deliver compassionate, patient-centered care to pediatric and adult patients in their own homes. Many of our patients have complex medical needs, including G-tubes, cerebral palsy, chronic conditions, and occasionally tracheostomy or ventilator support. You’ll partner with patients and care teams to carry out personalized care plans that promote health, independence, and quality of life, while meeting Indiana Medicaid requirements.

Key Responsibilities

All Nurses (RN and LPN):

• Conduct comprehensive head-to-toe assessments and carry out personalized care plans.
• Deliver hands-on care, including medication administration, wound care, and chronic disease management. Depending on patient needs, this may also include G-tube feedings, trach care, and ventilator support.
• Educate and empower patients and caregivers on disease management, medication regimens, self-care, emergency response, and therapeutic techniques.
• Coordinate care with physicians, therapists, case managers, and other members of the interdisciplinary team.
• Keep accurate, timely, and compliant documentation in the electronic health record (EHR), in line with agency, state, and Medicaid standards.
• Uphold the highest standards of safety, infection control, and patient privacy.
• Approach every patient interaction with professionalism and genuine empathy.
• Advocate for patient needs and connect patients with community resources and support services.

Shift Information

• Shifts range from 4 to 12 hours, tailored to patient needs and nurse availability.
• Flexible scheduling for day, evening, and weekend shifts.
• Choose consistent long-term assignments or varied schedules.

Qualifications

• Current, unrestricted Registered Nurse license in Indiana.
• At least 1 year of nursing experience; home health and Medicaid experience strongly preferred.
• Solid understanding of Indiana Medicaid policies, documentation standards, and prior authorization processes.
• Active LPN license in Indiana, or an active compact LPN license.
• Minimum of 1–2 years of clinical nursing experience; home health and pediatric experience strongly preferred.
• Experience with G-tube, trach, or vent care is a plus (training provided).

All Candidates:

• Current CPR certification.
• Sharp clinical judgment and confident independent decision-making.
• Strong communication, interpersonal, and organizational skills.
• Valid driver’s license and reliable transportation.
• Familiarity with electronic documentation systems preferred.
