Duties

• Assess patients’ physical conditions by conducting comprehensive evaluations of mobility, strength, and functional abilities.
• Develop and implement tailored treatment plans utilizing evidence-based physiotherapy techniques such as trigger point therapy, gait training, manual therapy, and therapeutic exercises.
• Monitor patient progress regularly and adjust care plans accordingly to optimize recovery outcomes.
• Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
• Collaborate proactively with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate care and discharge planning.
• Educate patients and their families on injury prevention, home exercise programs, and community resources to support ongoing recovery.
• Manage discharge processes effectively by preparing comprehensive reports and ensuring seamless transitions from inpatient or outpatient settings to home or community care.

Requirements

• Valid state licensure as a Physical Therapist with practicing credentials in good standing.
• Proven experience working with diverse patient groups including geriatrics, pediatrics, neurological cases, and orthopedics.
• Strong knowledge of physiology, anatomy, medical terminology, and clinical rehabilitation techniques.
• Demonstrated ability to develop effective treatment plans based on thorough assessments and patient goals.
• Experience with acute care, inpatient hospital settings, outpatient clinics, home health services, or outpatient rehabilitation centers.
• Proficiency in electronic health records (EHR) management and EMR systems for accurate documentation and reporting.
• Excellent client communication skills with a compassionate approach to patient care.
• Ability to work collaboratively within multidisciplinary teams to ensure comprehensive patient management.
• Knowledge of discharge planning processes and familiarity with outpatient rehabilitation protocols.
• Additional skills such as orthopedics expertise, neurology specialization, physiotherapy techniques like trigger point therapy, and experience working with geriatric or pediatric populations are highly desirable.

Pay: $45.00 - $50.00 per hour
