• Provide direct patient care by monitoring vital signs, administering medications, and performing routine assessments with precision and compassion.
• Assist with complex procedures such as IV infusion, catheterization, wound suturing, and sterile technique to ensure optimal patient outcomes.
• Support patient mobility and safety through proper use of equipment like Hoyer lifts and ventilators while adhering to safety protocols.
• Document all patient interactions accurately using Point Click Care
• Collaborate with multidisciplinary teams on discharge planning, case management, and coordination of post-acute or home care services.
• Conduct specialized assessments including neurological exams, respiratory evaluations, and pain management to inform treatment plans.
• Educate patients and their families on health coaching topics such as medication administration, nutrition, dementia care, Alzheimer's care, and managing chronic conditions like diabetes or infectious diseases.

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification in Basic Life Support (BLS).
• Proven experience in diverse clinical settings such as medical-surgical units, ICU, emergency room, or long-term care facilities.
• Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), and diagnostic evaluation procedures.
• Skilled in performing phlebotomy, injection administration, tube feeding management, and specimen collection/processing.
