Responsibilities

• Provide direct patient care and implement RN care plans

• Document visits accurately and timely

• Educate patients and caregivers

• Communicate changes in condition to RN

• Follow physician orders and company policies

Qualifications

• Active NM LPN license

• Current CPR certification

• Valid driver’s license & reliable transportation

• Strong communication and documentation skills
