Licensed Practical Nurse - Charge Nurse

Job Overview

Our facility is under NEW management and are looking to hire LPN charge nurse for all shifts (6am-2pm, 2pm-10pm, 10pm-6am). We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the role of Charge Nurse, leading patient care teams with enthusiasm and expertise. In this vital position, you will oversee daily nursing operations within a healthcare facility, ensuring the delivery of high-quality, patient-centered care. Responsibilities

• Lead and coordinate nursing staff to deliver comprehensive patient care aligned with facility protocols and standards.
• Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical documentation within Point Click Care
• Administer medications, injections, IV infusions, and perform catheterizations following aseptic techniques and safety guidelines.
• Oversee clinical procedures including wound care, tube feeding, ventilator management, and sterile processing.
• Collaborate with interdisciplinary teams on discharge planning, case management, and diagnostic evaluations to optimize patient outcomes.
• Ensure compliance with long-term care regulations, HIPAA confidentiality policies, and infection control protocols.

Qualifications

• Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
• Proven experience in long term nursing facilities.
• Demonstrated proficiency with Point Click Care
• Strong knowledge of medical terminology, physiology, pharmacology, and long-term care regulations.
• Experience working with diverse patient populations including geriatrics, individuals with developmental disabilities, or those requiring hospice or memory care services.
• Skilled in patient assessment techniques such as vital signs monitoring, specimen collection/processing, and pain management.
• Excellent communication skills for effective caregiver collaboration and client engagement.
