Licensed Practical Nurse

Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse to provide high-quality care to clients in their homes. The successful candidate will have a solid clinical background, demonstrate exceptional clients care skills, and appreciate the opportunity to work flexible hours while positively impacting clients’ lives.

Responsibilities

• Provide direct patient care in clients' homes under RN supervision and per the established care plan.
• Perform basic nursing care, including wound care, medication administration, and other treatments as ordered.
• Monitor and record vital signs, symptoms, and changes in client condition.
• Assist in implementing and updating individualized care plans as directed by the supervising RN.
• Support and coordinate with CNAs/home health aides as needed.
• Educate clients and family members on basic care, medication adherence, and wellness practices.
• Maintain a clean and safe environment by following infection control protocols.
• Accurately and promptly document all nursing care provided in the client's medical record.
• Report any changes in client condition promptly to the supervising RN.
• Follow all agency policies, procedures, and compliance standards.

Qualifications

• Active LPN license in Georgia.
• At least 1 years of nursing experience (home care preferred but not required).
• Current CPR/BLS certification.
• Excellent organizational and communication skills.
• Strong clinical assessment and critical thinking skills
• Flexible and willing to work in a growing agency environment.
• Reliable transportation for in-home visits
