Duties

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using Electronic Health Record (EHR) systems.
• Administer medications, injections, IV infusions, and treatments such as tube feedings and catheter care with precision and adherence to safety protocols.
• Assist with diagnostic evaluations, specimen collection/processing, and medical imaging procedures while maintaining sterile technique and infection control standards.
• Monitor patients’ conditions closely, manage acute pain, airway management, ventilator support, and respond promptly to urgent or emergent situations such as trauma or cardiac events.
• Collaborate with interdisciplinary teams on discharge planning, case management, and long-term care regulations to optimize patient outcomes across settings like skilled nursing facilities or hospice care.
• Maintain accurate medical documentation in compliance with HIPAA confidentiality policies and ICD coding standards (ICD-9/10) for billing and recordkeeping purposes.
• Provide education and support to patients and their families on health coaching, medication administration, dementia or Alzheimer’s care, and other specialized needs like pediatric or neonatal care.

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN) with current CPR/BLS certification.
• Proven experience working with diverse populations including geriatrics, individuals with developmental disabilities, or those requiring memory or behavioral health support.
• Demonstrated proficiency in EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks; familiarity with medical terminology and ICD coding is highly preferred.
• Critical care experience in settings like ICU or Level I Trauma Centers is advantageous; experience in emergency medicine or urgent care is a plus.
• Strong knowledge of anatomy, physiology, infection control practices, and long-term care regulations to ensure safe patient assessments and interventions.
• Excellent communication skills for caregiver collaboration and client education; ability to work independently while following clinical confidentiality policies.
• Flexibility to adapt to various clinical environments including hospital medicine, outpatient clinics, home & community care settings or travel nursing assignments.
