About the Role:

A Joyful Adult Health Care Center is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our caring team in Woburn, MA. Responsibilities:

• Provide direct patient care including wound care, medication administration, and vital sign monitoring
• Assist in developing and implementing individualized patient care plans
• Collaborate with RNs, physicians, and interdisciplinary care teams to ensure optimal patient outcomes
• Document patient assessments, treatments, and progress accurately in medical records
• Monitor and report changes in patient condition to supervising nursing staff or physicians
• Educate patients and families on health management, medications, and care procedures
• Maintain a safe, clean, and compliant clinical environment in accordance with state and federal regulations

Requirements:

• Active Licensed Practical Nurse (LPN) license in the state of Massachusetts
• Previous experience in adult health care, geriatrics, or a similar clinical setting preferred
• Strong knowledge of medication administration and infection control protocols
• Excellent communication and interpersonal skills with patients, families, and staff
• Ability to work effectively in a team-oriented, fast-paced environment
• CPR/BLS certification required or ability to obtain upon hire
• Compassionate, patient-focused attitude with a commitment to quality care
