Responsibilities

• Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation using Electronic Health Record (EHR) systems (PCC)
• Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
• Assist with diagnostic evaluations such as specimen collection, processing, and basic medical imaging procedures.
• Manage complex care needs including wound care, catheterization, tube feeding, and pain control strategies.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with conditions such as dementia, Alzheimer's disease, or developmental disabilities.
• Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting medical records accurately and thoroughly.
• Provide patient education on health management topics such as medication administration, nutrition, first aid procedures, and discharge planning.

Requirements

• Valid state licensure as a Licensed Practical Nurse (LPN).
• Proven experience working with geriatric patients in settings like skilled nursing facilities or long-term care environments.
• Familiarity with EMR/EHR systems such as Epic, Cerner or Athenahealth for documentation and patient management.
• Knowledge of ICD-9/ICD-10 coding for accurate medical documentation and billing purposes.
• Experience in specialized fields such as hospice care, infectious disease management
• Strong communication skills for effective caregiver-patient interactions and team collaboration.
• Ability to perform physical examinations, venipuncture (blood draws), sterile techniques, and aseptic procedures confidently.
• Certification in Basic Life Support (BLS) and CPR is required; additional certifications like IV insertion or advanced wound care are advantageous.

Pay: $24.50 - $27.50 per hour
