Job Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties

• Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
• Administer medications and injections accurately following prescribed protocols, including IV infusions and catheter care.
• Manage acute pain and assist with complex medical procedures such as tube feeding, venipuncture, and sterile dressing changes.
• Utilize electronic health record (EHR) systems like Epic or Cerner to document patient information securely and efficiently.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with conditions such as dementia, Alzheimer's disease, or developmental disabilities.
• Provide specialized care for geriatric patients in settings like skilled nursing facilities or memory care units, ensuring compliance with long-term care regulations.
• Support emergency response efforts through first aid, airway management, and CPR in urgent situations like trauma or acute illness episodes.
• Assist in discharge planning and client education to promote ongoing health management at home or in community settings.

Experience

• Valid Licensed Practical Nurse (LPN) license with current certification to practice in your state.
• Experience working with diverse patient populations across multiple healthcare environments such as hospitals, nursing homes, outpatient clinics, or home care.
• Proficiency in EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate medical documentation.
• Demonstrated competence in critical care areas such as ICU or Level I Trauma Center settings is highly desirable.
• Prior experience with specialized nursing procedures like IV insertion, phlebotomy, sterile technique, and medication administration.
• Knowledge of medical terminology, anatomy, physiology, and ICD coding standards (ICD-9/10).
• Strong communication skills for effective caregiver communication and patient education within multidisciplinary teams.
• Familiarity with infection control policies and HIPAA regulations to ensure patient confidentiality and safety.

Pay: $26.00 - $28.00 per hour
