Position Summary:

The LPN provides direct nursing care to residents under the supervision of an RN or physician, ensuring the highest standards of care in a skilled nursing environment.

Responsibilities:

• Administer medications and treatments as ordered
• Monitor and document residents’ conditions and report changes
• Provide wound care and other clinical interventions
• Supervise and support CNAs
• Ensure compliance with state and federal regulations
• Maintain accurate and timely documentation

Qualifications:

• Active LPN license in [State] or Multi State License
• Previous experience in a skilled nursing facility preferred
• Strong clinical and communication skills
• Ability to work in a fast-paced environment
• Compassionate and team-oriented mindset
