Licensed Practical Nurse

The role involves providing resident-centered clinical care in a long-term care and post-acute setting.

The Licensed Practical Nurse (LPN) will

• administer medications and treatments
• monitor resident conditions
• accurately document in the EHR
• support assessments and implement care plans
• educate residents and families

Working within an interdisciplinary team , the LPN will uphold the organization’s faith-based, mission-driven culture by practicing servant leadership , maintaining dignity and safety , and engaging in continuous learning and quality improvement .
